This report originally defined quality using the STEEP (safe, timely, effective, efficient, equitable, person-centered) dimensions.  It highlights that despite vast advances in medical knowledge and technology, the health-care system still falls high-quality care. It calls for a fundamental redesign of health care delivery that is less system-centered and more patient/client-centered.
